Brazil Visa for India passport holder living in Singapore

Visa application form: Two fully completed and signed application form (do not print double sided). This form must: be completed in block capitals only and be signed in blue/black ink only.

Original Passport : valid at least six months from the date the application is lodged and has at least two blank visa pages.

Passport-type photo: One passport sized colored photograph.

Proof of status. Employment Authorization or Residency Permit: a copy (front and back) of your Employment Pass, Blue IC, Work Permit or "PR".

Bank Statement. Your last three month's original bank statements, plus photocopies. Your current salary, full name and bank account number must be visible on the bank statement. Copies are accepted with the bank's original endorsement stamp.

Letter of Introduction. If you currently employed, you must supply an original company letter. The letter must:

Hotel booking. Сopy of the hotel confirmation. Also you must include a copy of the daily tour itinerary.

Itinerary. Copy of an itinerary or e-ticket showing entry and exit dates. Show the traveler's full name as it appears in the passport.

Yellow Fever Certificate. One copy of yellow fever vaccination certificate, the certificate must cover the intended length of stay in the destination.
